In the unlikely case of no blastocyst formation, OVACY Egg Bank will replace with six frozen oocytes (same or new donor depending on availability), once per recipient and free of charge without any further guarantee, provided the below sperm quality criteria* are fulfilled.
*Criteria to qualify for the OVACY Egg Guarantee Program:
• The sperm used in the fertility treatment must contain a minimum of 5 million/ml in the final sperm preparation with normal morphology (≥4%).
• The sperm must be assessed at the time of egg warming immediately prior to ICSI.
• The fertility treatment must take place at an OVACY Egg Bank approved fertility center.
• The fertility treatment must take place within 12 months of the initial treatment.
